Purpose: Delivers adaptive equipment, home modifications, behavior support, therapy, and caregiver training via Home & Community-Based Services (HCBS) to Medicaid-eligible youths at risk of institutional placement.

Persons With Disabilities (PD) Waiver

ISBE Special Education & Tech Assistance
